Cattle Pregnancy Test — Blood Test, Ultrasound and Rectal Palpation Explained

Method 1 — Rectal Palpation

Rectal palpation is the traditional cattle pregnancy check — the method most ranchers mean when they say "the vet is coming to preg-check." A veterinarian or experienced technician inserts a gloved, lubricated arm into the rectum and manually palpates the reproductive organs through the rectal wall.

What the practitioner is feeling for:

  • An enlarged uterine horn (the side where the fetus is developing)
  • A fluid-filled embryonic vesicle (detectable from about day 35)
  • The fetus itself, which becomes palpable from around day 60–70
  • A corpus luteum on the ovary (indicating a maintained pregnancy)
  • The characteristic 'membrane slip' feel of placentomes from about day 75

Rectal palpation is practical, requires no equipment beyond gloves and lubricant, and can be done in a basic working chute. Accuracy from an experienced practitioner is high from day 40+ onward. The limitations are practitioner dependence (inexperienced palpation can be inaccurate or, in rare cases, damaging to early pregnancies) and the inability to determine fetal sex or assess embryo quality.

Method 2 — Ultrasound

Portable ultrasound machines have become increasingly affordable and are now common on many ranches. Transrectal ultrasound — a probe inserted rectally and directed toward the reproductive tract — allows the operator to visualize the uterus, embryo, and fetus in real time.

Ultrasound offers capabilities that palpation cannot:

  • Earlier detection: Embryonic sacs and heartbeats are visible from day 25–28
  • Embryo viability: A heartbeat confirms the embryo is alive; the absence of a heartbeat in an otherwise detectable sac indicates embryonic death
  • Twin detection: Twins are identifiable from the early embryonic stage
  • Fetal sex determination: From approximately day 55–75, the genital tubercle can be visualized to determine fetal sex — valuable for operations focused on heifer development or bull marketing
  • Gestational aging: Fetal measurements can estimate gestational age when the breeding date isn't precisely known

Method 3 — Blood PAG Test

The blood pregnancy-associated glycoprotein (PAG) test is the most modern cattle pregnancy confirmation method and has become widely adopted in large commercial operations. PAGs are proteins produced exclusively by bovine placental tissue — they appear in the cow's bloodstream from approximately day 28–30 after breeding.

The test is done by collecting a blood sample (usually via tail vein) on-farm. Samples are refrigerated and shipped in batch to a commercial lab, which returns results within a few days. Cost per sample is low when processed in bulk.

One important limitation: PAGs can persist in the bloodstream for several weeks after pregnancy loss (embryonic resorption or abortion). A cow that was pregnant but lost the pregnancy in the previous 60–90 days may still test positive on the PAG test. This creates false-positive situations — a cow reads pregnant but isn't. Labs typically flag samples from cows tested within 90 days of calving or pregnancy loss. Combining PAG testing with ultrasound confirmation is the most reliable approach for herds with this concern.

  • Small operation, limited budget: Rectal palpation from your large animal vet is the most accessible and affordable option.
  • Large commercial herd, need bulk testing efficiently: Blood PAG testing processed through a lab is the most scalable and cost-effective.
